    I love Tap Forms! I am a locum tenens palliative care consultant at a local hospital and came up with with a form that I use to keep track of the patients I see for billing purposes. I can filter charges pending, charges completed, and a list of active patients that are new and that I am following. I have a date of service form that is integrated to the patient form where I can keep up with cpt codes, dates I see patients and follow ups and it keeps up with my time spent so that I can turn in time sheets for billing. The pick lists like cpt codes and time spent are customizable to your own setting, as this can be used by any healthcare provider that needs to keep up with their own billable hours.

    Hi Eddy,

    I’m a Palliative Care doctor in Australia. I see patients at 2 hospitals and struggle to keep up with billings. Your efforts have given me a head start designing my own form.

    The attached files to this 9+ year old post (as of my post) by Eddie Koonce will not open in TapForms 5 for the Mac. I would be very interested in updated files or in similar capabilities if anyone has done a project like this. I will contact Eddie via his link above to see if he can update the files in this post.

    Hi Steve,

    Try using the Import Form Template command in the File menu Tap Forms 5 to import the attached files.

    Worked for me.
