Is there a Movie database template, like the one shown in the image on the Mac App Store

Jus adding to my own message, I’ve realised that the User Guide says that there should be a set of sample templates, but I can’t find those anywhere

They’re in the Sample Forms document that gets installed the first time you launch Tap Forms.
