Just recently had incident where our cell bill was unexpectedly high. Reading thru all the cell phone promotions can get confusing, at least to me. This template helped sort it out — maybe it will be helpful to others.
Provider Plans will create two forms, one where you enter the information, a second that will present the data in a way to show what the breakdowns are.
