I’ve called this accident report but it’s really a collision report form.
Not that I’m planning on having a crash but if I do it should be handy to whip out my phone and be reminded of all the information I need to collect for my insurance company. Third party details, photos, location & witness details etc
Oh dear, same result as before, file was 8kb before upload!

Testing uploads. Seems to work for me. I used a different (non admin) user account to make sure that I didn’t have any special privileges for uploading. So I’m really not sure why it’s not working for you.
What if you zip the file first and then upload it?
